Radiofrequency Catheter Ablation of Ventricular Tachycardia in Patients Without Structural Heart Disease

چکیده
Background. Radiofrequency energy has been used safely and successfully to eliminate accessory pathways in patients with the Wolff-Parkinson-White syndrome and the substrate for atrioventricular nodal reentrant tachycardia. However, this form of ablation has had only limited success in eliminating ventricular tachycardia in patients with structural heart disease. In contrast, direct-current catheter ablation has been used successfully to eliminate ventricular tachycardia in patients with and without structural heart disease. The purpose of this study was to test whether radiofrequency energy can safely and effectively ablate ventricular tachycardia in patients without structural heart disease. Methods and Results. Sixteen patients (nine women and seven men; mean age, 38 years; range, 18-55 years) without structural heart disease who had ventricular tachycardia underwent radiofrequency catheter ablation to eliminate the ventricular tachycardia. Two patients presented with syncope, nine with presyncope, and five with palpitations only. Mean duration of symptoms was 6.7 years (range, 0.5-20 years). Radiofrequency catheter ablation successfully eliminated ventricular tachycardia in 15 of 16 patients (94%). Sites of ventricular tachycardia origin included the high right ventricular outflow tract (12 patients), the right ventricular septum near the tricuspid valve (three patients), and the left ventricular septum (one patient). The only ablation failure was in a patient whose ventricular tachycardia arose from

Radiofrequency catheter ablation for ventricular tachycardia.
Radiofrequency energy was used for the ablation of chronic recurrent ventricular tachycardia (VT) in 58 patients who were divided into two groups: 44 cases with structural myocardial disease (36 men and 8 women: mean age 55 years; range: 14 to 85 years) with an average left ventricular ejection fraction of 38% (range: 15 to 80%): these patients had myocardial infarction (28 cases), arrhythmogen...
متن کاملRisks and Benefits of Catheter Ablation of Ventricular Tachycardia in Patients with an Implantable Cardioverter-Defibrillator
Patients with an implantable cardioverterdefibrillator (ICD) may receive frequent shocks or antitachycardia pacing for monomorphic ventricular tachycardia despite suppressive drug therapy. Antitachycardia pacing is often well-tolerated but not always effective. High voltage cardioversion is usually painful and may cause severe, disabling anxiety for some patients. Therefore, catheter ablation i...

OBJECTIVES The aim of this study was to assess the clinical characteristics and the efficacy of radiofrequency catheter ablation (RFCA) for idiopathic ventricular fibrillation (VF) and/or polymorphic ventricular tachycardia initiated by ventricular extrasystoles originating from the right ventricular outflow tract (RVOT). BACKGROUND Ventricular fibrillation and/or polymorphic ventricular tach...
متن کاملRelation between efficacy of radiofrequency catheter ablation and site of origin of idiopathic ventricular tachycardia.
The results of radiofrequency catheter ablation of ventricular tachycardia (VT) in patients without structural heart disease are reported. Particular attention was focused on the relation between efficacy and the site of origin of the VT. Eighteen consecutive patients (5 women and 13 men; mean age 41 +/- 13 years) with idiopathic VT underwent catheter ablation using radiofrequency energy. Sites...
متن کاملAdenosine-sensitive ventricular tachycardia. Clinical characteristics and response to catheter ablation.
BACKGROUND Sustained ventricular tachycardia in the absence of structural heart disease may have diverse mechanisms. Termination of the tachycardia by adenosine suggests triggered automaticity as the etiology in many of these patients. We examined the clinical characteristics, electrophysiological responses, and results of catheter ablation in this patient subgroup. METHODS AND RESULTS Intrav...
